There are again wide-ranging causes for the main reason of skin ageing. Besides genetic factors, long-term contact with sunlight and one’s own lifestyle have an enormous influence on the “perceived age of the skin”.
Ageing in itself might not be a profound problem if it were not for the external effect on other people. The slackening of muscles and the sagging of fatty tissue make faces look tired, weary and less confidence-inspiring. What are the procedures for a facelift?
For an individual facelift, we offer the most innovative options currently available in plastic and aesthetic surgery in Munich and throughout Germany. Depending on your individual findings and expectations, a forehead lift can already lead to the desired results, such as smoothing out the unloved frown lines. If the wrinkles predominate in the cheeks, a cheek lift with tightening of the underlying tissue can help.
In a classic face-neck lift, the region of the cheeks and temples as well as the neck are treated in one operation. Since the deep wrinkles around the mouth can be practically levelled at the same time with a classic face-neck lift, it has proven to be worthwhile to carry out this procedure in addition with a skin-rejuvenating laser dermabrasion. The exact planning of the operation will be carried out together with you in our practice for plastic and aesthetic surgery at the Marienhof in Munich after you have been thoroughly informed about the options available.
What should be considered before a facelift?
A surgical procedure must always be planned and performed with the utmost care. Therefore, it is absolutely necessary that you are in the best of health when we perform an invasive facelift. However, not only your health condition plays an important role, but also the interactions with stimulants. Therefore, we always ask you to keep nicotine and alcohol consumption to a minimum before an operation.
Under no circumstances should you take aspirin or similar medicines in the seven days before the procedure! The reason for not taking these drugs is their inhibitory effect on blood clotting. In addition, we would like to advise you to plan about seven days off after the operation. After the operation, you will still be limited in your abilities for the first time, and freedom from stress will also improve recovery.
Information always comes before a facelift!
Before every operation, Dr von Isenburg, Dr Neuhann-Lorenz or Dr Haas will provide you with comprehensive information. For you, this means asking all the questions that concern you. It is helpful to take notes beforehand. Don’t be afraid to ask everything that interests you. If any problems or fears remain, we will be happy to clarify them by telephone or at another personal preliminary consultation.
All key data about the operation and your personal data are then recorded in the consent form. Particularly important for you: the specification of the type of operation and documentation of all potential complications addressed. Your signature on the declaration guarantees that you have received the appropriate information from our team and that you agree to the operation with knowledge of the risks.
Are there any risks with a facelift?
Complications with a facelift are rather rare in our practice in Munich. As with any operation, bruising and swelling can occur, which usually heal on their own. The very rare secondary bleeding with considerable swelling and pain must be surgically repaired. We will take care of this. Infections of the wound area are also very rare and can usually be healed quickly with appropriate antibiotic therapy.
Wound healing disorders are equally rare, whereby factors usually play a role that are discussed in detail in the conversation before the operation. The main reasons for delayed healing are risk factors such as those found in diabetics, smokers and patients with blood clotting disorders.
Temporary numbness of the skin in the surgical area may occur due to the severing of the finest skin nerves. In the course of healing, sensitivity generally returns to normal. Overall, the face lift is a particularly low-complication operation with a high degree of efficiency and safety.
What kind of narcosis is used?
Most facelifts are performed under local anaesthesia and twilight sleep, so that you are also asleep during the entire operation. In addition to the local anaesthetic, you will be given a tranquilliser (sedation) which puts you into a kind of twilight sleep. You then breathe on your own, but are relaxed and insensitive to pain.
Alternatively, face lifts are performed under general anaesthesia so that you are deeply asleep and ventilated throughout the operation. In both cases you will be examined by an anaesthetist before the operation to clarify your fitness for anaesthesia. Regardless of the method, the anaesthetist is present throughout the operation and controls the anaesthesia.
How does a SMAS facelift work in the practice of Drs. von Isenburg & Neuhann-Lorenz?
A facelift in our plastic surgery practice in Munich has the following procedure:
1 Guidance of the skin incision on both sides in a curved line within the hairy scalp.
2 Lifting of the skin from the underlying layer of muscle and fat.
3 Modelling of the excess tissue of the neck, chin and cheeks to improve contours.
4 Tightening of the muscle and fascia tissue of the neck and face (SMAS) for more lasting effect.
5 Removal of the excess sections and careful closure of the incisions.
6 Insertion of fine drainage tubes behind the ear before final wound closure.

The fine drainage tubes allow escaping blood or wound secretions to drain easily in the first post-operative phase, i.e. 12 to 24 hours after the operation. The head bandage counteracts post-operative swelling in the first few days and thus accelerates healing.
What should be considered after the facelift?
Immediately after the procedure
In the first few days after a facelift you will feel slight pain in the entire surgical area. As a result, you will be somewhat limited in your facial expressions and in the mobility of your head. These after-effects are alleviated by medication prescribed by us. The fine drainage tubes are usually removed on the first day after the operation. You should leave the head bandage in place for about 5 days. We recommend that you keep your head still and straight for the first few days after the operation. The less movement, the less swelling!
In the first weeks to note
Any swelling, bruising and possibly numbness of the skin usually disappears completely in the following weeks. The first skin sutures are removed about five to seven days after the operation. You will be able to resume your normal daily activities in a few days. Nevertheless, you should always plan for periods of rest during the first week after the operation. After consultation with us, sports are allowed again after about 2 to 4 weeks.

Tips for optimal results
Special tip after the facelift: Reduce your nicotine and alcohol consumption to a minimum even postoperatively and avoid sauna and steam bath visits for a few months! Due to the swelling, your facial features may change a little and your facial expressions may appear somewhat stiff. The scars still change a lot over time, the initially red scars fade more and more until only narrow white lines are visible.
By the end of the second week, you usually look recovered and relaxed, and most patients start working again ten days after the procedure. If you wish, the remaining scars can be covered with special camouflage make-up.

Can a face lift be repeated?
In principle, a facelift can be repeated without problems, usually after about a decade has passed. Some patients deliberately choose the option of repeated touch-ups at shorter intervals.
