Often women suffer from their breasts being too small in relation to their body image. Although this does not lead to physical problems, it does lead to psychological pressure that should not be underestimated.
The daily look in the mirror is difficult. The woman feels disadvantaged. Another problem: sexuality and emotional experiences with the partner suffer. Surgical breast augmentation is a good possibility to adjust the breast, which is perceived as too small, to the desired body image. Depending on the individual silhouette, different expectations must be taken into account.
Goals of the change
Most women want naturally enlarged breasts that match their body. Other women want very large breasts. It should be made clear that not every request can (and should) be fulfilled for surgical-technical reasons. The reasons for breast augmentation are manifold. In addition to juvenile (congenital) breast hypoplasia, there is also often a need for a shape-improving surgical measure in women after breastfeeding.
After pregnancy and breastfeeding, the volume of the breasts is significantly reduced due to the involutional atrophy of the mammary gland. In this case, the sagging of the breast skin in the upper breast area leads to the impression of a hanging, drooping breast. These patients, who are often still young, suffer from the premature aging process that this causes. Usually excellent results can be achieved through volume compensation, as the skin mantle of the breast is already pre-stretched. The best conditions for an aesthetically good result are provided by women who have a harmoniously proportioned breast and for whom an implant or the patient’s own fat is only used to increase the good breast shape. Further breast shape improvements are possible, but usually only with additional scars. If an implant is used, it should be noted that the implant can almost always be palpable on close examination. In particular, the soft mantle at the bottom of the breast is not so well developed on the outside, so that the implant edge is most likely to be palpable here.
Which implants are available for Breast Augmentation?
All implants for breast augmentation consist of a shell of silicone. Only the ingredients can be different. Currently, implants are offered with silicone gel or physiological saline solution. Different sizes and shapes are available.
Silicone gel implants, which are superior to other implants in terms of consistency and variety of shapes, are still the most widely used. Due to the development of the cohesive silicone gel, on the one hand a “leakage” of the gel is hardly possible anymore, on the other hand a stable shape in the sense of anatomically shaped implants has become possible.
Saline implants contain a physiological saline solution which is based on the body’s own composition. With most manufacturers, the table salt is first injected into the implant intraoperatively. They have the advantage of being completely reabsorbable in the event of implant rupture, but do not feel as natural as silicone gel implants.
What are the risks of a Breast Augmentation with implants?
Complications are rare in breast augmentation and are usually insignificant for a lasting good result. However, in addition to the general risks of surgery, specific complications associated with the peculiarity of breast augmentation may occur in exceptional cases.
Risk age of the implant
Implants are also subject to an ageing process. However, a routine change of an implant is not necessary without further abnormalities. Clinical and modern radiological examination methods allow an exact evaluation of your implant in your body.
In rare individual cases, an implant defect may occur – whether due to external force or abrasion. These defects are often noticed by the change in the shape or firmness of the breast. In any case, a defective implant together with the surrounding capsule should be surgically removed. From a medical point of view, there are no objections to the insertion of a new implant.
Thirty years ago, the safety of silicone gel implants was doubted because some women complained of joint pain, fever, fatigue and breast pain after silicone gel implants were implanted. Many scientific studies have now proven that these were single cases.
Although an intolerance cannot be ruled out, there are no fundamental medical concerns about the insertion of silicone gel implants. All in all, breast augmentation is a low complication surgical method with a high degree of efficiency and safety.
General surgery risks
As with any operation, hematomas and swelling can occur, which usually heals by itself. Infections of the wound area are very rare and can usually be treated with suitable antibiotic treatment. Wound healing disorders are equally rare. Here, factors that were discussed in detail in the conversation before the surgery usually play a role. These include delayed healing in diabetics, smokers and patients with blood coagulation disorders.
Temporary numbness of the skin in the area of the operation may occur as a result of severing of the finest skin nerves. Sensitivity usually returns during the healing process.
Some women also notice a hypersensitivity or numbness of the nipples, a disorder that usually disappears completely as well. The ability to breastfeed is not restricted by a breast implant.
Mammographic breast cancer screening should be performed, although the evaluation of a mammography may be limited by the implant. Of course, the implants should be pointed out before the examination. This allows the examining doctor to use the correct diagnostic tools, such as ultrasound or magnetic resonance imaging.
Risk capsule fibrosis
The most common problem after breast augmentation with an implant is capsular fibrosis. The body recognises every implant (regardless of its content) as a foreign body and forms a connective tissue envelope around this foreign body. Usually this connective tissue capsule is so thin that it cannot be felt.
Occurrence
Capsule fibrosis describes a contraction of this connective tissue capsule. This can occur even years after the operation. The implant then feels firm or even hard. If pain occurs or there is an aesthetic impairment, surgery is required. The “scarred” capsule is removed and a new implant is inserted.
Avoidance
During the operation, we ensure that the risk of capsular fibrosis remains as low as possible. During the operation we work as atraumatically as possible. This means that we work quickly, accurately and avoid secondary bleeding and infections. In addition, we recommend individual follow-up measures in cases of risk.
What kind of narcosis is used?
Breast enlargements are usually performed under general anaesthesia, so that you sleep during the entire surgery. For this purpose, you will be examined by an anaesthesiologist before the operation to clarify your suitability for anaesthesia. The anaesthesiologist is present throughout the whole operation and controls the narcosis.

1 Personal information
Prior to any medical treatment, our team provides the patient with comprehensive information about the planned procedure and its risks. It may be helpful for you to make a note of your questions in advance. Do not be afraid to ask anything that interests you. Inform yourself, if necessary by telephone or at another preliminary meeting, if any problems have remained open.
The declaration of consent documents the patient’s name and date of birth (as well as that of the legal guardian if the patient has not yet reached the age of majority), the type of treatment and any complications mentioned. Your signature guarantees that you have received the appropriate information and that you agree with your treatment or that of your child, knowing the general and specific risks involved.
2 Preparation for treatment
Especially before a planned surgical procedure such as breast augmentation, you should ensure that you are in the best possible physical condition. For optimal healing and best possible results, it is recommended that you limit your nicotine and alcohol consumption beforehand. If you (have to) take the contraceptive pill or hormone replacement drugs, their use could be temporarily stopped. However, we will clarify this individually with you in a preliminary consultation.
Under no circumstances should you take Aspirin or similar medication in the last seven days before breast augmentation. It is also advisable to avoid them for the time after the operation.
3 Process of the surgery
The operation usually takes one to two hours. Which individual method of breast augmentation is performed for you depends on your anatomical conditions and your expectations as well as on the recommendations of your surgeon.
The skin incision of about 4 cm length is placed either in the breast fold or on the edge of the nipple or in the armpit. Wherever the incision is made, we make every effort to position it as unobtrusively as possible. Usually the scars heal very unobtrusively.
Through the skin incision, the preparation of the implant cavity takes place, which lies either behind the mammary gland (subglandular) or behind the breast muscle (submuscular, subpectoral). The correct positioning of the implants is usually done in a semi-seated position of the patient. Before the wound is closed, thin drainage tubes are inserted into the implant cavity, through which blood and wound secretion drain off in the first postoperative phase (12 to 24 hours). A firm dressing fixes the implants and compresses the surgical site.
What should be considered after Breast Augmentation surgeries?
Directly after the surgery
After the surgery you will feel pain due to the tensed chest. Pain-relieving medication is usually provided. The drains inserted during the operation are usually removed on the first day after the procedure. The bandage will be removed in the following days.
A specially fitted bra will support your healing process over the next two to three weeks. Eight to ten days after the operation the skin thread is removed. Initially, the skin scar is reddened and firm; after a few months, the scar becomes paler and fades with the surrounding area.
Restrictions & recommendations
Approximately one to two weeks after the breast augmentation, you should plan for relaxation and little stress, as you will have limited energy. After that, we recommend that you gradually resume normal everyday activities, apart from individual circumstances. Although the success of the surgery is already evident shortly after the procedure, it will still take a few months before the final result is achieved. Only slowly the breast will soften.
Hyper- or hyposensitivity of the nipples may require protection of the nipples against touching in the first weeks. It can also take months before the usual feeling is achieved again. Scar care can be carried out with suitable ointments from the third week after the operation.
Important to consider!
Reduce your nicotine and alcohol consumption to a minimum also postoperatively and avoid sauna and steam bath visits for a few months! Sun exposure of the scar should be avoided for the first 6 months. Protect the scar when sunbathing with a plaster. In any case, you should stop taking Aspirin or blood-thinning medication at least seven days before the procedure.
Can a Breast Augmentation be repeated?
Generally breast augmentations can be repeated. Regular check-ups in our clinic help to detect and treat any complications that may occur at an early stage.
